Form Processing Notice

To ensure fair and timely processing for all shareholders, Sunny Slope Water Company processes forms on a first-come, first-served basis.

• Cutoff Date: Applications received after October 31 will be processed starting in January of the following year.
• No Expedited Processing: There is no option to pay a fee or otherwise bypass the queue.

This policy allows us to manage the volume of requests and maintain consistent service for all shareholders. Thank you for your understanding and cooperation.

Address Change form: if you want to change the address where the bill is sent, add a tenant or remove a tenant, please print the Address Change Form below. Fill it out completely and return it to our office.

Auto Debit Application: if you would like to enroll in our Auto Debit program, please print this Auto Debit form, fill it out completely, and return it to our office.

Auto Debit Update Form: If you have changed banks or accounts, please print the form, complete it, and return it to our office.

Auto Debit Cancellation  Form: If you plan to cancel the Auto Debit program, please print the form, fill it out completely, and return it to our office.

**New Meter or New Service Form: if you need a new meter installed or to make changes to your existing service, please print the form, complete it, and return it to our office. Forms submitted after October 31st will be processed after January 1st of the following year.

**Will Serve Letter Form. Fill out the form and submit it. Application fee is $35. Forms submitted after October 31st will be processed after January 1st of the following year.

**Single Family Dwellings. Fill out the fire flow form. The application fee is $35. If the hydrant needs to be flowed, each hydrant is $500. Forms submitted after October 31st will be processed after January 1st of the following year.
